Request for Letter of Invitation:

Conference attendees requiring a letter of invitation can request once the online registration and the abstract submission are completed.
Please submit your request via email to with the following information.

1. Your full name
2. Degree (M.D., Ph.D., etc.)
3. Job Title
4. Your Registration ID beginning with "16"
5. Title of your Abstract
6. Organization/Institution
7. Your email address
8. Your mailing address* (if you require the original letter sent by mail)

*A PDF copy of your letter of invitation will be sent to you via email to the address provided. If you require the original letter of invitation mailed to you, please include your mailing address (including your country and postal code) in your request.
